Changing table structure through pivoting

I am a student new to Rapidminer may be missing something but I have the following simple data set

ID              | DESCRIPTION
21033207| AMIGDALECTOMIA COM ADENOIDECTOMIA
21033207| MIRINGOTOMIA COM INSERCAO DE TUBO
21033209 | OPERACOES NO VITREO NCOP
21033209 | INJECCAO DE SUBSTITUTO DE VITREO
21033209 | REPARACAO DE DESCOLAMENTO DE RETINA COM DEPRESSAO ESCLERAL E IMP
21033209 | INSERCAO PROTESE INTRAOCULAR CRISTALINO SUCEDENDO A EXTRACCAO CA
21033209 | FACOEMULSIFICACAO E ASPIRACAO DE CATARATA

Each description corresponds to a surgical procedure, My original intention was to create a new column saying if it was performed (alway saying yes) and then pivot it and replace the missing values with No so I could have some thing like this.
ID              |  AMIGDALECTOMIA COM ADENOIDECTOMIA | MIRINGOTOMIA COM INSERCAO DE TUBO | INJECCAO DE SUBSTITUTO DE VITREO|etc...
21033207| Yes                                                                  |   Yes                                                               |    No                                      

If I do that I only get a single column with and ID and other just saying Yes, I can see the logic behind that result but then how would I obtain the column I suggested?

    Nevermind found it, I just had to uncheck the "skip constant attributes button"
